CGI will take care of it!

This is a bit of a favorite subject of mine. Because I think a movie or video game like this should always be 18+.
And not every nipple should mean a movie is 18+ (and isn't in many regions, by the way).
It always about context.
Should sexual content be 18+? Of course!
But explicit violence should also be rated the same way.
